MoTER Remodel was designed to assist individuals to remotely manage their diabetes care. The system is comprised of a smartphone app, a Bluetooth-enabled glucose meter, a clinician portal, and a decision support tool. The app enables users to upload their BGL readings automatically via Bluetooth or via manual entry to the clinician portal via the internet. The app also supports a review of BGL and trends and entering of free-text notes. Automated messages (optional to the user) are triggered when users have severely low or high BGL values. All users also receive regular, automated messages based on BGL values and the frequency of BGL self-monitoring twice a week. Positive feedback messages are sent if parameters are within clinically recommended targets. If not, messages serve as prompts to check BGLs as recommended, to consider reasons for out of range BGL measurements, and to seek medical advice if required. Messages contain links to the national Diabetes Australia website for additional information regarding self-management.
